If a student intends to play sports for the Timberland Wolves, he/she must have a valid physical on file in the Activities Office prior to participation. Your Physical Examination & History Forms must be uploaded here. If you do not have a scanner available to upload a PDF of these forms, please bring the forms to the Activities copies so that we can upload them to your student's registration profile.
Reminder: A valid physical is dated after Feb. 1, 2016 (for the 2016-17 school year) and must be filled out and signed by a doctor.
All forms must be completed here on-line & your Physical Examination & History Forms uploaded. This must be completed TWO WEEKS PRIOR TO THE START OF EACH SPORTS SEASON.
3. COMPLETE THE IMPACT TEST
A student must be ImPact Tested every two years (This means, Freshmen, Juniors or students new to Timberland or new to athletics). There will be several dates throughout the year, please click here for the most current list of dates.
4. PAY ANY OUTSTANDING FINES PRIOR TO THE FIRST DAY OF PRACTICES/TRYOUTS
